VBS Registration Fort Macleod Alliance Church

This year’s Vacation Bible School will run from 9am to 12pm for kids ages 4-7 and from 9am to 2:30pm for kids ages 8-12.
To register your child, please email the following information to vbsfortmacleod@gmail.com STARTING MAY 1st*:
1. First and last name
2. Age and date of birth
3. Gender and health care number
4. Name of parent/guardian and home and cell phone number
5. Emergency contact and phone number and relationship to child
6. Health concerns/allergies
7. T-shirt size (youth or adult XS/S/M/L/XL)
